Home Study Documents

One of the first things you will begin working on, after you have selected your adoption agency is your adoption home study. The home study can be overwhelming for some parents, because in addition to required home visits, you will also need supporting documentation.

The State that you reside in has jurisdiction over your adoption process. Laws and requirements vary from state to state.  Your home study provider will provide you with a list of documents that you will need for your home study process. The following list will give you a good idea of what you may need:

  • Adoption decree (for previously adopted children)
  • Autobiographical statement
  • Birth certificate
  • Child abuse and neglect clearances
  • Criminal background check
  • Divorce decree
  • Driver’s License
  • FBI clearance
  • Financial information
  • Green Card
  • Immunization records
  • Income tax return and tax returns for the last 2-3 years
  • Marriage certificate
  • Military discharge(s)
  • Parenting plan statement
  • Passport
  • Proof of insurance
  • Physician Statement of Good Health (medical form)
  • Physician’s statement addressing current medical diagnosis
  • Pet vaccination records
  • Social Security Card
  • Sex offender clearance
  • Verification of employment
